The pastor (or moderator, when applicant is the pastor) must validate the applicant's membership and affirm the suitability of the applicant for leadership in the Conference at this time.

I understand that my continued service on my leadership team requires my participation in the background disclosure process. This will include a review of my personal and professional background, including potential conflicts of interest and a review of my criminal history. In addition, depending on my disclosure, I may need to participate in an Active Review process and provide relevant details to the committee to more fully understand my qualification to serve. I understand that a criminal background check is required for my continued service and that one will be ordered for me upon my submission of this form.
